The name, position, dates and place of the committed Government's employee's work with facts of Corruption, Collusion and Nepotism or any other type corruption in the Government's operations. |

A summary of the facts that explain as the Government's employee made corruption with other to obtain goods or the Government's in an illegal way services. |

The letter should be containing the complainer's data to be contacted, being guaranteed the confidential nature of the information. However, OIG respects the anonymity, since the claim is truthful. |
